How Can We Help?

How much will my copayment be?

Copayments vary for each insurance plan. I encourage you to either call your insurance company, or call our office, and we would be happy to look into this for you!

What is your new patient procedure?

When you call to schedule an appointment, we will ask for your email to send you the new patient paperwork. If you do not have access to the internet, that's okay as well. We can give you the same paperwork when you check in at your appointment.

I don’t have my insurance card with me, can I still receive treatment?

We will gladly provide treatment. We will simply bill you as a private pay until you are able to bring in your insurance card. Once we have your insurance card, we will then submit your visit to your insurance for reimbursement.

Do you offer any Mental Health Services?

Yes — we proudly offer specialized mental health services. Crystal's primary focus is mental health medication management and counseling support while providing comprehensive, individualized care for each patient.
